The older adult may experience low back pain for different reasons than their younger generation. Rather than suffering from disk herniation or facet syndrome, the older adult can suffer from disk dehydration, resulting in “neuroforamen” – narrowing of the openings through which the nerve root exits the spine. Multiple conditions can contribute to this neuroforamen, some of which include: narrowing and bulging of the disk, osteoarthritis, or spurring extending off the vertebral endplates where the disk attaches, facet joint arthritis resulting in “hypertrophy” or enlargement, calcification of ligaments, and more.

Symptoms begin to occur when the neuroforamen shrinks to a point of pinching on the nerve, this condition is referred to as spinal stenosis. The literal definition of spinal stenosis is “narrowed spinal canals” with entrapment of the spinal cords and/or nerves. Low back pain and stiffness are the most common symptoms related to spinal stenosis. Spinal stenosis gradually restricts mobility, limiting the time a person can spend walking. 

Bending forward can help reduce the symptoms related to spinal stenosis, as bending forward opens up the neuroforamen. This is why you may see patients suffering with spinal stenosis leaning forward as they walk, making this their norm.

The predecessor is osteoarthritis. Common symptoms related to osteoarthritis include: morning stiffness, stiffness and pain when rising from sitting, decreased range of spinal motion or flexibility, localized painful joints, and others.
